I understand and agree that the DWR may use the name, voice and/or picture of the named Minor in any media coverage of the Event or in promotion of its Wildlife Recreation Program as matches my choices in this form. I understand that this document remains effective for the duration of the Slam, unless expressly revoked, and that this document will be kept and maintained by the Utah Division of Wildlife Resources.
